Attending team planning meetings and phone consultations to ensure consistent communication.
Reviewing DDS/Regional Center documents and correspondence to keep you informed.
Analyzing individual needs and desires to identify current obstacles and opportunities.
Coordinating with your multidisciplinary team, including your Service Coordinator, service providers, and Financial Management Services (FMS), to ensure seamless support within both SDP and traditional services.

Scheduling and facilitating meetings to clearly identify your needs, goals, and desired services.
Developing detailed PCP reports that highlight your strengths, capabilities, preferences, lifestyle, and cultural values. These reports are crucial for informing and shaping your Individual Program Plan (IPP).
I specialize in helping you maximize the benefits of the Self-Determination Program, offering hands-on support with:
Spending Plan Development: This includes clarifying SDP service code definitions, outlining steps to acquire a certified budget, and explaining how your Spending Plan is created.
12-Month SDP Budget Management: You receive dedicated, ongoing support to ensure you are confidently and effectively using your SDP funds and leverage generic resources.
Budgeting & Expenditure Tracking: You will gain a clear understanding of your budget and learn to track expenditures with ease, giving you full control over your funds .
Service & Support Coordination: This involves assisting with finding and hiring service providers or agencies, and negotiating staff pay or provider rates to align with your SDP budget.
Staff Management Support: I offer basic information on provider qualifications and payment rules, and help you manage your staff (e.g., setting expectations, managing hours, and coordinating timely submissions with FMS and the Regional Center).
